Attention! 
Certain payment types will require a PDF attachment with supplementation information.  Please DO NOT upload any documentation that contains personal identifiable information (PII) such as a social security number. Black out the PII before uploading the PDF attachment in Pay.gov.

Jury Duty Reimbursements - This form is used only for civilian employees who served on jury duty and who were on pay status. If you served in jury duty and were on leave status during this time, you do not have to reimburse for the jury duty pay that you received. Please email testagency@email.gov if you have any questions.
