The DFAS Contractor Debt Payment 5570 form is used for the submission of funds where on contract where the payment office is listed as HQ0105, HQ0335, HQ0431, HQ0374, HQ0471, HQ0301, HQ0346, or HQ0345 and the Demand Letter states to send the funds to DFAS-IN AR.

Effective December 4, 2017, customers will no longer be allowed to submit credit card payments to repay "debt" owed to the government via Pay.gov. This will include any of the available payment methods that can be linked to a credit card (i.e. Amazon Pay). Bank branded debit cards, however, will still be an acceptable payment method.

Accepted Payment Methods:

  • Bank account (ACH)
  • PayPal account
  • Debit or credit card

  • Copy a form you already submitted the next time you need to make a payment.
