About this form
Use this form to pay your SBA serviced loan payments. Examples include COVID-19 Economic Injury Disaster Loans (EIDL), Regular Disaster Loans, and Paycheck Protection Program (PPP) Loans Purchased by SBA.
Attention! The Pay.gov portal and SBA CAFS portal do not share Login credentials.
How do I make a Payment? The 1201 Borrower Payment form can be used for one-time payments as well as recurring payments.
- Recurring payments - You will need to create an account on Pay.gov and sign in before completing this form. Once signed in, search 1201 to go back to this form to begin the process. You must select bank account (ACH) as your payment method. Click here for detailed instructions on creating your recurring payment
- One-time payment - You will need to complete this form each time you pay. Signing into a Pay.gov account is optional.
What information do I need to make a payment? You will need the loan information below BEFORE you complete this form to make a payment:
- Loan number
- Monthly payment amount
- Next payment date
- For recurring payments, you will need the term of your loan to calculate the number of payments
